I, the undersigned, do hereby consent and agree that photographs, videotape, and/or digital recordings of me and my child(children) may be used by Lyonsword Publishing, including but not limited to all owners of Lyonsword Publishing, in any and all media, now or hereafter known, and exclusively for the purpose of business social media and on the website and blog of author Ondrea Keigh. I further consent that my name and identity and my child’s(children's) name(s) and identity(identities), may be revealed therein or by descriptive text or commentary.

I do hereby release to Lyonsword Publishing, including but not limited to all owners of Lyonsword Publishing, all rights to exhibit this work in print and electronic form publicly or privately. I waive any rights, claims, or interest I may have to control the use of my own or my child’s(children's) identity(identities) or likeness(likenesses) in whatever media used.

I understand that there will be no financial or other remuneration for recording myself or my child(children) and allowing recorded media of myself and/or my child(children) to be used by Lyonsword Publishing, either for initial or subsequent transmission or playback.

I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
